NHS Pay Rise April 2026: Your New Salary Starts Now – What to Check on Your Payslip
The 3.3% NHS pay award for 2026/27 is now in effect. Your April payslip should reflect the new rates. Here's your complete guide — new band salaries, take-home figures, pension impacts, and exactly what to do if your pay still looks wrong.
Today is the day. From 1 April 2026, every member of NHS Agenda for Change (AfC) staff in England receives a 3.3% pay increase — the earliest confirmation of a pay award in years. For most trusts, you'll see the new rate appear in your April payslip, paid at the end of this month.
Unlike previous years where industrial action and delayed negotiations meant confusing backpay calculations stretching into autumn, the early announcement this time means you should be paid at the right rate from day one — with no backpay headaches.

What Will You Actually Take Home?
Gross salary headlines are useful, but the number that matters is what lands in your bank account after income tax, National Insurance, and your NHS pension contribution are deducted. Here's a rough guide for the most common bands:
Band 2 (Top)
Gross: £25,272
~£1,740/ month
Basic rate tax. ~5.1% NHS pension.
Band 5 (Entry)
Gross: £32,073
~£2,120/ month
Basic rate tax. ~6.1% NHS pension.
Band 6 (Entry)
Gross: £39,959
~£2,550/ month
Basic rate tax. ~7.1% NHS pension.
Band 7 (Top)
Gross: £56,515
~£3,180/ month
Higher rate kicks in. 40% tax on amounts above £50,270.
The Frozen Threshold Trap (Fiscal Drag)
The Personal Allowance remains frozen at £12,570 and the Higher Rate threshold at £50,270 — no change for 2026/27. This means:
- Band 5 & 6: Most of your increase is taxed at 20% + NI — you keep roughly 67–70p in every extra £1.
- Band 7 top & Band 8: Your salary now exceeds £50,270. The entire increase may be taxed at 40%, meaning you net only ~55p per £1 of rise.
London Weighting & High Cost Area Supplements
The 3.3% uplift applies to High Cost Area Supplements (HCAS) too. Projected new figures for Inner London are:
| Area | 2025/26 Min | 2025/26 Max | 2026/27 Min (est.) | 2026/27 Max (est.) |
|---|---|---|---|---|
| Inner London | £5,327 | £8,088 | £5,499 | £8,352 |
| Outer London | £4,069 | £5,019 | £4,203 | £5,185 |
| Fringe | £1,304 | £2,011 | £1,347 | £2,077 |
What to Check on Your April 2026 Payslip
When your payslip arrives (usually the last working day of April), run through this checklist:
Basic Pay Line
Your "Basic Pay" should reflect the new 2026/27 rate for your band and increment point. Compare it to the NHS Employers published tables.
Increment Date
If your personal increment date is in April, you may get a double increase — both the 3.3% cost-of-living uprating AND a step progression.
Pension Contribution Rate
NHS Pension contribution rates are tiered by salary. Because your gross pay is higher, your contribution band may have nudged up slightly. Check your payslip's pension deduction line.
London / HCAS Supplement
If you receive HCAS, check this has also been uprated by 3.3%, not left at the old figure.
Tax Code
Confirm your tax code is correct (most will be 1257L). An emergency tax code (W1/M1) would mean you're being taxed incorrectly — contact HMRC if so.
What If My Payslip Doesn't Reflect the New Rate?
- Don't panic — some trusts may apply the increase to the May payslip with backpay for April if there were processing delays.
- Contact your HR or Payroll department directly, referencing "2026/27 Agenda for Change pay uplift."
- Check your Trust's intranet or ESR (Electronic Staff Record) for any posted pay notices.
- If unresolved, raise a formal query through your union rep (RCN, Unison, BMA as applicable).

3 Smart Things to Do With Your Pay Rise
Review your budget — but only after tax
A 3.3% gross increase ≠ 3.3% more in your pocket. Work out your actual new monthly take-home before adjusting any standing orders or direct debits.
Check if you can increase AVC pension contributions
If you pay into an AVC scheme alongside the NHS pension, topping this up is highly tax-efficient — especially for higher-rate taxpayers.
Update any finance applications
If you're applying for a mortgage or remortgage, make sure your broker and lender are aware of your updated salary — it could improve your borrowing capacity.
